Feedback & complaints

Tell us how we're doing

Good, bad or in between — we want to hear it. Fill in whichever parts apply. You can stay anonymous.

You do not have to use this form and you do not have to use the word 'complaint'. You can tell either of us directly, call, text or email, or ask a family member, friend, advocate or support person to help you.

If anyone is in immediate danger or there is a life-threatening emergency, call 000. Do not wait for this form to be read.

You can contact the NDIS Quality and Safeguards Commission directly at any time on 1800 035 544. You do not have to raise it with us first.

What you want to tell us

We take this seriously. You won't be treated differently for raising it.

Safety
Is anyone in immediate danger right now?

If anyone is in immediate danger or there is a life-threatening emergency, call 000 now. Submitting this form is not an emergency response.

Does this concern possible abuse, neglect, exploitation, violence, sexual misconduct or another risk of harm?

Best way to reach you

What you send here goes to a separate feedback mailbox, kept apart from enquiries and session notes. If you raise a safety concern, we may need to pass it on to keep someone safe. We use what you tell us to understand your feedback, respond to any concern, keep safe, keep the records we are required to keep and improve what we do. We handle complaint information as confidentially as we reasonably can, but we cannot promise absolute secrecy — information may need to be shared where necessary for safety, fairness to a person a complaint is about, legal duties, professional advice or an external authority such as the NDIS Commission. Complaint details are kept in our restricted complaints records, not in ordinary session notes. If you stay anonymous we cannot get back to you, but we will still look into it. See our privacy page.
